Hospital Indemnity

What Is It?: Hospital Indemnity provides a fixed daily case benefit If you are admitted and/or confined to the hospital, regardless of hospital fees or insurance coverage, Plan participants would receive $1,000 for hospital admissions(1 day per plan year), and an additional $1,000 for admission to the ICU (1 day per plan). Additionally, plan participants would receive $150/day for hospital confinement (up to 364 days per plan year) and an additional $150/day for ICU confinement (up to 364 days per plan year).

Who's it available to?: Crescent provides ALL benefits eligible employees with a 100% Employer Paid Hospital Indemnity Benefit. Employees also have the "Buy Up" option to include spouses and children.

Marquee Health Portal

What Is It?: Marquee Health portal is a great tool for all things wellness-related - wellness challenges, individual coaching programs, the My Wellbeing Blog, and most importantly, your Biometric Screening form that you need to complete in order to be one of our Wellness Partners and receive a discounted healthcare rate. Critical Illness

What Is It?: Critical Illness provides a fixed benefit if you or your covered dependent are diagnosed with cancer, heart attack, stroke, end stage kidney disease, major organ failure, and more. Benefits paid will not be impacted by any other insurance you have.

Who's it available to?: Crescent provides ALL benefits eligible employees and their spouses and children with an employer paid $5,000 Critical Illness benefit. Employees also have a buy-up option for additional coverage.

Accident Insurance

What Is It?: Accident insurance is voluntary, employee-paid, policy that provides a fixed benefit when you have an off-the-job accident. There is a schedule of reimbursement, which are paid directly to you, for fracture, dislocations, emergency room visits, x-rays, physical therapy, and more! They even have a $50 Wellness Benefit for completing your annual wellness visit with your PCP, Benefits paid will not be impacted by any other insurance you have.

Cigna - Dental

What Is It?: Crescent Offers a voluntary dental plan through Cigna, where the employee pays 50% and Crescent pays the other 50% premium. You can use their website to find a dentist in your area.

Please note: Cigna Dental does not issue personalized insurance cards - If you are a member and need a "generic" ID card, please contact Diane or Jenny.

What Is It?: Crescent employees have the opportunity to enroll in Cigna's vision program - vision insurance and premiums are 100% EMPLOYER PAID.

New York Life - Life Insurance

What Is It?: Crescent provides employees with a group life and accidental death and dismemberment (AD&D) benefit of $50,000, and pays the full cost of this benefit. Employees who want to supplement their group and AD&D insurance benefit by purchasing additional coverage.

What Is It?: Crescent provides employees with long-term disability income benefits and pays the full costs of this coverage. In the event you become disabled from a non-work-related injury or sickness, disability income benefits are provided as a source of income.

PAS

What Is It?: Personal Assistance Services (PAS) is Crescent's employee assistance program (EAP). PAS is a life event service that helps you and your dependent family members with virtually any life need. They have an extensive array of counseling and coaching services to help you thrive in life. EPIC Retirement Plan Services

What Is It?: Crescent provides employees with a 401(K) plan in order to save for retirement. Crescent makes matching contributions equal to 20% of the first 10% salary defer. The annual interest and dividend earnings of the fund are added to each employee's account based on a proportionate interest in each fund. The fund becomes available to employees upon reaching various vesting levels. See the Benefit'sSummary for more details.

EPIC Retirement Plan Services gives you access to your investment elections portfolio, account overview, and quarterly elections, current portfolio, account overview, and quarterly statements online. Additionally, they have a Retirement Education Center which offers tools and education for all facets of investing and saving for your retirement. Clean Uniforms

What Is It?: Crescent offers employees an opportunity to purchase company-branded clothing through Clean Uniform and individual employee uniform accounts. Employee uniform accounts are comprised of employee and employer contributions.

Through payroll deductions, employees can elect to pay a $2/day period (26 pay periods/year), and Crescent will make a match of $4/pay period, amounting to an annual employee uniform account balance of 156$/year. Crescent also makes an additional one-time contribution of $50 at the time of hire.

What Is It?: Through IdShield, Crescent employees have the option of "buying up" and adding LegalShield protection. This benefit is 100% employee-paid. Your LegalShield membership includes legal advice on personal legal issues, letters/calls made on your behalf, contract and document review, attorneys to prepare your will, living will and health care power of attorney, moving traffic violations, uncontested divorce, separation, adoptions, and/or name change representation, IRS audit assistance, and 24/7 emergency access for covered situations.

Education Incentive & Investment Policy 


What Is It?: Crescent recognizes that the educational and professional development of employees is important to its success and is willing to invest in it. Crescent wishes to encourage loyal and hardworking employees to complete educational courses and training so that they can have the tools to increase their competency in their present jobs and prepare them for possible advancement in the future. In order to receive the full tuition benefit, employees must have a five-year period of continuous employment. Employees must complete an educational pledge and promissory note.

Computer Loan


What Is It?: Crescent provides an interest-free company loan to employees who wish to purchase a personal computer, laptop, or tablet for home use. The loan is for two(2) years and is paid back through payroll deductions (52 payroll periods).

Paycom

What Is It?: Pay com is the web-based software Crescent Uses to Process payroll and complete other necessary HR functions. Employees can use Paycom's Employee Self-service software to self-manage certain transactions with 24/7 access, including clocking in/out (non-exempt employees only), requesting use of PTO, updating personal information(such as address and contact information), viewing your pay stubs, complete compliance training and secure paperwork, and enroll in benefits during Crescent's Open Enrollment Period.
